Adult Heart Transplant Program

Provides transplants for people who have heart failure and meet the program's eligibility criteria.

Provides transplants for people who have end stage lung only or heart-lung organ failure, and who meet the program's eligibility criteria.
The Transplant Program provides assessment for patients with end stage organ failure, which includes but is not limited to:

  • medical, surgical and psychosocial assessment
  • diagnostic investigations
  • consultation with specialists

If a patient is deemed suitable for organ transplant, the Transplant Program will:

  • list the candidate for organ transplant
  • monitor the candidate alongside the primary care physician while listed for transplant
  • provide transplant education and support the candidate through the transplant experience

If a patient receives an organ transplant they will be followed by a Transplant Program lifelong.

Eligibility

Patients are assessed by the heart transplant team using tests and interviews to see if they’re good candidates for surgery. Clients are considered for a heart transplant if it’s likely the surgery will help the person live longer and have a better quality of life.

Clients are assessed for:

  • how urgent their need is
  • health and medical history
  • medical test results

Clients should:

  • be able to follow a complex medical treatment plan
  • be emotionally stable
  • have social support
  • have realistic attitudes about their illness and the transplant surgery
