Active Rehabilitation Program - Mental Health

This program offers a structured environment suitable for patients with chronic mental health illnesses, such as psychotic disorders.

The program is for those who can take part in their rehabilitation and gradual transition to community setting.

Services may include:

  • medication teaching / management
  • group therapy
  • psychosocial progamming
  • crisis management
  • discharge planning (transitional housing referrals, mental health referrals)

Service Providers May Include
dietitians, licensed practical nurses (LPNs), mental health aides, occupational therapists (OTs), psychiatrists, psychologists, recreation therapists/assistants, registered nurses (RNs), registered psychiatric nurses (RPNs), social workers
Eligibility
  • adults 18 years and older
  • no ongoing or upcoming legal matters
  • the following disorders do not fall within the program mandate of the Concurrent Disorders Program: Schizophrenia, Schizoaffective Disorder, Psychotic Disorder, Substance Induced Psychotic Disorder, Psychotic Disorder Not Otherwise Specified, Amnestic and other Cognitive Disorders
Referral Needed

A referral from a psychiatrist is required. The referral form can be downloaded from the Claresholm Centre for Mental Health & Addictions website: www.ahs.ca/claresholmcentre
