Provides team-based care to support the health and well-being of patients in a number of areas including healthy eating, active living, chronic disease management and mental health support. The Edmonton North PCN also has on-site specialty services including women’s health, minor surgery, dermatology, internal medicine and physical medicine and rehabilitation. All services require a referral from a family physician who is a member of the Edmonton North PCN.
